To enroll your child in Mile Creek School, you must reside in the Town of Old Lyme, Connecticut. Tuition Students are also accepted if space allows, but approval must first be granted through the Office of the Superintendent of Schools (860-434-7238).

* Pupil Registration form
* Residency Affidavit
* Potassium Iodide (KI) Administration Authorization
* State of Connecticut Health Assessment Record
* Certificate of Medical Exemption (for those students who are exempt from vaccinations due to medical reasons)
* Certificate of Religious Exemption (for those students who are exempt from vaccinations due to religious beliefs)
