Enrolling Your Child
To enroll your child in Lyme School, you must reside in the Town of Lyme, Connecticut. Tuition Students are also accepted if space allows, but approval must first be granted through the Office of the Superintendent of Schools (860-434-7238).
Please contact Mrs. Kimberly Sullivan, School Secretary, at 860-434-1233 to obtain registration forms. Forms are also available by clicking FORMS. The forms needed to enroll your child are as follows:
* Pupil Registration form
* Residency Affidavit
* Potassium Iodide (KI) Administration Authorization
* State of Connecticut Health Assessment Record
* Certificate of Medical Exemption (for those students who are exempt from vaccinations due to medical reasons)
* Certificate of Religious Exemption (for those students who are exempt from vaccinations due to religious beliefs)
The following additional form is also requested of new kindergarten students:
* Parents as Partners form
The following additional form is also needed for students who transfer from other schools:
* Permission for Release of Academic Records form
An original birth certificate (with raised seal) must also be presented at the time of registration.
